Observations and Corrective Actions: <br />25. Pharmaceutical waste is stored onsite for longer than 90 days. <br />- A person may store pharmaceutical waste onsite for not longer than 90 days when the container is ready for disposal. HSC <br />118280(f) — Pharmaceutical waste has not been picked up from10/25/19 to 2/14/19. Ensure pharmaceutical waste is <br />picked up within 90 days. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) — A liner was not placed in the biohazard containers in the <br />outside storage area. Line the containers with biohazard bags, provide photographic evidence to the EHD. <br />Submit all corrections to the EHD by March 16, 2020. <br />NOTES: <br />1. Stericycle is the medical waste hauler. Biohazardous waste is picked up every 7 days. <br />2. Tracking records were available for review. <br />3. The pharmaceutical waste pick-up frequency has decreased. The waste was picked up approximately 1 to 2 times a <br />month in the beginning of 2019, now it is picked up within 90 days. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
