HSC117960—Submit revised MWMP by 11/19/2015. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and tracking <br /> documents for 2 years. <br /> - A large medical waste generator required to register with the EHD shall maintain individual treatment operation <br /> records and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. <br /> HSC117975(a)—From 3/5/15 forward,this facility has been in compliance with this regulation.No corrective <br /> action needed. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> -Biohazard bags containing medical waste which will be sent offsite for treatment shall be marked and cetified by the <br /> manufacturer as having passed ASTM D 1922 and ASTM D 1709. When collecting medical waste within the facility, <br /> the waste may be placed in a biohazard bag which meets ASTM D 1709 provided that when the bag is prepared for <br /> transport offsite, it is placed into a USDOT-approved container line with a biohazard bag that is ASTM D1709 and <br /> ASTM D1922 certified. HSC 117630—Submit evidence of compliance by 11/19/15. <br /> Notes: <br /> 1.) Facility contracts with Stericycle for the removal of medical waste. Medical waste is picked up every <br /> Thursday. A review of this facility's treatment and tracking documents revealed an 8 day period between <br /> pickups from 4/2/15 to 4/10/15. This facility conducted an investigation with Stericycle and determined that <br /> the hauler did not have enough time to pickup the waste resulting in the late pickup. Facility provided an <br /> investigation report. Ensure biohazardous waste is removed every 7 days. <br /> 2.) Facility provided medical waste tracking forms for the hauling of medical waste from satellite locations to this <br /> facility.
