The Medical Waste Management Plan <br /> submitted to the EHD requires updating with the following information by 1/21/2019: <br /> (b)The business address for the facility <br /> (d)The types, and the estimated average monthly quantity, of medical waste generated for the facility <br /> (e)The plan should be updated to reflect that no treatment occurs at the facility. <br /> (f)The name and business address of the registered hazardous waste hauler used by the generator to have <br /> untreated medical waste removed for treatment, if applicable, and, if applicable, the name and business address <br /> of the common carrier transporting pharmaceutical waste pursuant to Section 118032. <br /> (g)The name and business address of the offsite medical waste treatment facility to which the medical waste is <br /> being hauled, if applicable. -A list of all Stericycle medical waste treatment facilities in the reqion does not <br /> meet this requirement. <br /> (k)A statement certifying that the information provided is complete and accurate. <br /> 13. This registered large medical waste generator does not maintain individual treatment records and <br /> shipping and tracking documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment <br /> operation records, and shipping and tracking documents for all untreated medical waste shipped offsite <br /> for treatment for 2 years. HSC117975(a) The facility only provided access to records for 20 months. Provide evidence of <br /> final treatment for medical waste shipped offsite for the records provided by 1/21/2019. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. <br /> HSC 117630 The small red biohazard bags in the storage area do not meet ASTM D1922 and ASTM D1709. The bags <br /> should be discarded immediately to prevent use. <br /> Notes: <br /> 1.) The biohazardous waste bags in use meet both the ASTM D1922 and the ASTM D1709 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
