M <br />The overall authority and direction of Lincoln Family Medical Group's Emergency <br />Action Plan (EAP) rest with the medical group's Administrator and his alternate <br />our Administrative Secretary, and in her absence <br />If the Administrator is not in the facility, at the time of the emergency, he <br />will be notified by telephone. <br />The person highest in the chain of command at the time of the notification will <br />assume responsibility of the facility until relieved. <br />Notification of an internal emergency will be directed to the Administrator/ <br />Designee from the affected area. External emergency notification will come by <br />telephone. The information should be recorded carefully and reported to the <br />Administrator/Designee. <br />It is the responsibility of each department head to ensure that all department <br />personnel have read and are familiar with the Emergency Action Plan. <br />A current recall roster will be maintained by each department head. The recall <br />roster will be updated quarterly. Each employee is responsible for reporting <br />any change in his/her phone number or address. The Administrative Secretary <br />will maintain the recall roster for departments heads and administrative staff. <br />_ to ee Responsibilities: <br />Each employee is personally responsible for being familiar with the contents <br />of the EAP, as well as his/her individual department responsibility. <br />Responsibilities of each department.are as follows: <br />Physicians and Nurses <br />Receptionists <br />Laboratory/X-ray/Pharmacy <br />Communications/Business Office/ <br />Medical Records/Transcription <br />Z- 7:7 ;FN M -:77 M r: Y •, z IF,' y F7 <br />03 1 Ft Yj FEE <br />- Evacuation,of all patient examination/ <br />treatment rooms and restrooms <br />- Evacuation of waiting areas <br />- Evacuation of respective departments <br />- Evacuation of your own work areas, assist <br />other staff as your availability permits <br />Administrator/Designated Business - Turn off gas lines or electrical power as <br />Office Representative/Receptionist appropriate <br />Upon hearing the page "ATTENTION ALL PERSONNEL, PLEASE EVACUATE THE BUILDING". <br />all employees on duty shall return to their departments and <br />follow instructions given to them. <br />It is the responsibility of off duty employees, hearing of the existence of mass <br />casualties or a disaster to report to work immediately. DO WAIT TO BE CALLED. <br />-1- <br />
