Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) — Biohazard containers were observed in the West Lab <br />and the East Lab without proper labels or with faded labels. — Corrected on site. <br />25. Biohazardous waste is stored above 32 F onsite for more than 7 days. <br />- A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br />onsite for more than 7 days. A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br />store that waste above 32 F onsite for more than 30 days. HSC 118280(e)(1) — Tracking records indicate biohazardous <br />waste was stored onsite for more than 7 days (from October 24, 2019 to November 1, 2019). "No one here" was written <br />on the tracking record for October 31, 2019 by the medical waste hauler. Ensure an explanation is documented for the <br />medical waste not picked up within 7 days. <br />31. The tracking records were not complete. <br />- Medical waste is tracked from the point when the waste leaves the generator facility until it receives final treatment. The <br />tracking document should include all information listed in HSC 118040 —The tracking record for April 25, 2019 did not have <br />the generator's signature. The volume of medical waste picked up on July 25, 2019 was not visible on the tracking <br />record. The information written on the August 29, 2019 tracking record was not visible. Ensure all tracking records are <br />complete and legible. <br />31. Sterilization operator training records not available. <br />The operators of the treatment equipment shall be required to receive training in the operation of the treatment equipment, proper <br />protective equipment to wear, if any, how to clean up spills, and other information required to operate the treatment equipment in <br />a safe and effective manner. <br />- Annual training for the operators shall be provided after the initial training has been completed. HSC 117938 — There are no <br />records indicating training was provided to operators of the sterilization equipment (autoclave). Provide training to <br />operators and management on an annual basis, submit documentation of training to the EHD. <br />Provide evidence of correction(s) to the EHD by March 16, 2020. <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
