DVI does generate some chemotherapy waste in the pharmacy. This stream of waste must be identified in the <br /> MWMP (in the section that describes the types of medical wastes generated). <br /> 2. Does DVI handle pharmaceutical wastes classified as DEA or controlled substances? I forget if you do or <br /> not. Either way it should be stated that you(or do not) in the MWMP. <br /> 3. The MWMP describes Stericycle's main location as "4136 W. Swift Ave." However the actual location is <br /> 4135 W. Swift Ave. That location is where they do their autoclaving (for biohazard and sharps waste). They <br /> also have an incineration facility for the chemotherapy and pharmaceutical waste at a different location. That <br /> address (should be stated on the manifests)needs to be stated in the MWMP too. <br /> Please make the above mentioned changes and resubmit. Contact me with any questions. The last time we spoke I was having problems <br /> with our camera to document our results. I have attached a copy of the CAP that was completed in January 2016. 1 also <br /> included the completed Medical Waste Plan. We are waiting for a signature from the Warden to make the document <br /> official. Next week I will forward pictures of the areas identified in your report. Thank you for your patience.
