The Medical Waste Management Plan does not <br /> have an estimate of the amount of medical waste generated per month. Submit an updated copy of the facility's <br /> Medical Waste Management Plan to the EHD by 2/12/19. <br /> 23. Biohazard bag does not meet the standard set forth in HSC 117630. <br /> -Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear-resistant. HSC 117630. The <br /> small biohazardous waste bags in use do not meet the ASTM D1709 and ASTM D1922 standards. Submit evidence of <br /> correction by 2/12/19. <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c). One of the biohazardos waste containers in use is not <br /> labeled on all lateral sides. (This violation was corrected during the inspection.) <br /> 29. Intermediate storage area is not secured or marked with proper signage. <br /> - Intermediate storage area shall be either locked or under direct supervision or surveillance and marked with the international <br /> biohazard symbol or the warning signs, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED <br /> PERSONS KEEP OUT", in English and in another language determined to be appropriate by the infection control staff or LEA. <br /> HSC118307. The supply storage room is marked with a biohazardous waste sign and is not used for biohazardous <br /> waste storage. Remove the biohazardous waste sign prior to use for regular storage. Submit evidence of correction <br /> by 2/12/19. <br /> All corrections are due by 2/12/19. <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for the facility. Biohazardous waste is picked-up once per week. <br /> 2) Annual training records are available for the 7 employees with medical waste contact. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
