Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> - Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words"Biohazardous Waste"or"BIOHAZARD"on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c). <br /> Three of the biohazardous waste containers in use is not labeled on all lateral sides. (This violation was corrected during <br /> the inspection.) <br /> Notes: <br /> 1) Stericycle is the licensed medical waste hauler for the facility. Biohazardous waste is picked-up once per week. <br /> 2) The file copy of the Medical Waste Management Plan is current. An updated attachment to the MWMP was provided <br /> during the inspection. <br /> 3) Annual training records are available for the 5 employees currently working in the facility. There are eight employees. <br /> 4) The biohazard bags in use in the facility meet both the ASTM D-1709 and ASTM D-1922 standards. <br /> Reinspecion onlabout: A reinspection fee of$152 per hour may be charged.
