HSC 117960 - LQG shall file MWMP with the EA. <br />OBSERVATION: The Medical Waste Management Plan (MWMP) does not contain all required elements. The MWMP is <br />missing an emergency action plan. <br /> <br />CORRECTIVE ACTION: Revise and submit the MWMP to the Environmental Health Department. The Medical Waste <br />Management Plan must include the following: <br />(a) The name of the person. <br />(b) The business address of the person. <br />(c) The type of business. <br />(d) The types, and the estimated average monthly quantity, of medical waste generated. <br />(e) The type of treatment used onsite, if applicable. <br />(f) The name and business address of the registered hazardous waste hauler <br />(g) The name and business address of the offsite medical waste treatment facility <br />(h) An emergency action plan complying with regulations adopted by the department. <br />(i) If applicable, the steps taken to categorize the pharmaceutical wastes <br />(j) A closure plan for the termination of treatment at the facility using <br />(k) A statement certifying that the information provided is complete and accurate. <br /> <br />#25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br />OBSERVATION: Biohazardous waste was stored on-site for more than 7 days. Waste was stored on site from 5/20/25 – 5/29/25 <br /> <br />CORRECTIVE ACTION: Ensure that biohazardous waste is stored onsite for no more than 7 days. A facility that generates 20 <br />or more pounds of biohazardous waste per month, shall not contain or store that waste above 0° Centigrade (32° Fahrenheit) at <br />an onsite location for more than seven days without obtaining prior written approval of the enforcement agency. <br /> <br />#30. HSC 118310 - Designated accumulation area shall be secured and marked with proper signage. <br />OBSERVATION: The designated accumulation room is not secured to deny access to unauthorized personnel. <br /> <br />CORRECTIVE ACTION: Install a lock to the designated accumulation room door. A designated accumulation area used for the <br />storage of medical waste containers prior to transportation or treatment shall be secured so as to deny access to unauthorize d <br />persons and shall be marked with warning signs on, or adjacent to, the exterior of entry doors, gates, or lids. The storage a rea <br />may be secured by use of locks on entry doors, gates, or receptacle lids. <br /> <br /> <br />Submit Evidence of Corrections to the EHD <br />By 09/21/2025 <br /> <br />NOTES: <br />1. Reviewed 10/8/24-08/18/2025 disposal records. <br />4. The facility is requesting to be reviewed and categorized as a small quantity generator. Submit monthly weight records <br />for the last 12 months for review.
