Foley, R.E.H.S. <br /> Dear Kasey Foley: <br /> Pursuant to your routine inspection of our facility on June 10, 2005,here is the <br /> information you have requested. <br /> I apologize for the delay. I was not able to meet the date of submission of this <br /> information by June 17, 2005 as you have requested. I was on vacation when you came <br /> for the inspection. When I was expected to come back, I had to take additional days until <br /> June 30, 2005 because I got sick with shingles and I was really very sick. I was in <br /> excruciating pain even up to this time. However, I have been able to drive and finally <br /> come to work. <br /> Our staff has started implementing the use of the Pharma Star containers for our <br /> pharmaceutical waste. We obtained the containers from Stericycle, Inc. The used <br /> containers have not been picked up yet,but they will be by July 5, 2005. Please see <br /> enclosed copy of the container label and the date we have started its implementation.
