0 <br /> Stericyde® The Specialist in <br /> Vo Managing Medical Waste <br /> November 1,2000 <br /> Dear Valued Stericycle Customer: <br /> The Tower conversion phase of the Stericycle,Inc.computer system is under way and some of these <br /> changes will effect you and your staff. Following is a review of these changes that will take effect <br /> November 1,2000 and we hope this letter will answer any questions you may have. <br /> • New account numbers—Each Customer will be receiving a new'Power system account number <br /> beginning November 1,2000. Please make a note of this change. <br /> • Biotrack®container labels—Stericycle'has a unique approach to the tracking process known as <br /> BIOTRACK®Bar codes and optical scanners or"readers"record all pertinent data required by the <br /> Medical Waste Management Act-MWMA. Labels are attached to each container for this purpose. <br /> Enclosed are the.new.Stericycle BIOTRACK®container labels for your use. No other label needs to <br /> be placed on the outside of your containers other than one BIOTRACKO label and the Biohazard <br /> labels that are required on all four sides of the container plus the lid. This BIOTRACK®LABEL <br /> MEETS ALL FACILITY IDENTIFICATION LABELING FOR FINAL PACKAGING <br /> REQUIREMENTS BY MWMA and Department of Transportation-DOT. Red bag and sharps <br /> container labeling is still required. Please note that the new label is GREEN in color and has a <br /> Stericycle watermark. <br /> All gther labels previously provided by Stericycle or BFI should be eliminated from your <br /> inventory and should not be placed on the outside of any container being picked up by our <br /> Stericycle drivers. <br /> Detailed Daily Activity Reports(RDA's)=The monthly poundage report is being eliminated from <br /> our new Tower system. Your monthly weights and container types will be provided with your <br /> monthly billing rather than sending a separate document. If your billing address is other than your <br /> service location you may want to contact your accounts payable department to ensure receipt of this <br /> information! <br /> We would like to thank you for your continued support during this transition period.
