Weber Ave., P Floor <br /> Stockton, CA 95202 <br /> RE: Medical Waste Inspection <br /> Violations Record ID: PR0521665 <br /> Dear Ms. Foley: <br /> This is in response to a visit from you on June 2, 2005 at the Gambro Healthcare — San <br /> Joaquin facility. We regret and apologize that the violations from June 24, 2004 were not <br /> corrected at the of the June 2 visit. The facility has had a complete change of <br /> management since that time. Every effort will be made to correct the current violations <br /> to your satisfaction immediately and we offer the following plan of correction for your <br /> approval. <br /> Alleged Violations: <br /> 1. Procedures #11: Medical Waste Management Plan was never submitted <br /> pursuant to letter received from Nicole Berg dated 1, 2004. There has been a <br /> change in Center Director and Clinical Administrator position has been <br /> eliminated. We are now submitting an updated Medical Waste Management <br /> Plan using the Guidelines for the Medical Waste Management Plan. Please <br /> find the plan enclosed. <br /> 2. Treatment # 16: We regret that records of Pharmaceutical waste tracking <br /> were incomplete. Going forward, the Center Director and Secretary will file <br /> all tracking slips. An in-service was given on November 2004, regarding <br /> proper labeling of pharmaceutical waste. These will be labeled, "incineration <br /> only" waste. Approximately 1 (one) container of pharmaceutical waste is <br /> generated per week and labeled"incineration only". Going forward,the Unit <br /> Secretary will maintain a copy of all invoices on file in the clinic for reference <br /> as needed We will submit invoices from Stericycle from 7/l/04—6/2/05 to the <br /> Environmental Health Department. Completion date: 6/9/05. <br />
