0 <br /> MEDICAL WASTE MANAGEMENT PLAN <br /> Gambro Healthcare San Joaquin <br /> June 2,2005 <br /> 1 List the types of regulated medical waste generated at your facility(include pharmaceutical waste). <br /> a.) Blood and body fluids <br /> b.) Sharps <br /> c.) Pharmaceutical waste <br /> 2. Estimate the monthly amount of each waste stream in pounds, of medical waste generated at your <br /> facility. <br /> a.) Autoclavable waste-250 lbs. <br /> b.) Incinerator waste - 20 lbs. maximum <br /> 3. Estimate the monthly amount of each waste stream in pounds, of medical waste generated at your <br /> facility: <br /> a.) We have a storage area located near the treatment area that is labeled as medical and <br /> biohazard waste. We have designated garbage receptacles with tight fitting lids, <br /> located on the treatment area with orange biohazard stickers on them and a red bag in <br /> them that will also designate that they are biohazard materials. These bags will be <br /> changed when they are 3/4frill and stored in the medical/biohazard waste area until <br /> pickup from hazardous waste hauler. Pharmaceutical waste is separated in white and <br /> blue containers that are kept separate from the biohazard waste but stored in the same <br /> area. <br /> b.) Medical and biohazard waste storage area is located near treatment area and is locked at <br /> all times. All containers that will hold waste will have tight fitting lids. We have a <br /> printed scheduled from the hazardous waste hauler for every Monday and Thursday. <br /> All containers in this area will have the Biotrack container labels that contain all <br /> pertinent information required by the Medical Waste Management Act—MWMA. <br /> c.) N/A <br /> d.) We have an agreement with Stericycle Medical Waste Systems, Inc.,28161 N. Keith <br /> Drive, Lake Forest, Illinois 60045 (559)275-0992. <br /> e.) Medical Waste Treatment Facility, 1345 Doolittle Drive, San Leandro, CA 94577, <br /> MWTF Permit#p-132, Treatment by autoclaving. <br /> f.) N/A <br /> g.) We are supplied with tracking documents from Stericycle, Inc.: Medical Waste <br /> Tracking Document,Autoclave Tracking Document-Biotrack. <br /> h.) We adhere to Gambro Healthcare Policies and Procedures. When a small spill occurs, <br /> we are able to contain with spill kits that we have throughout the facility. For larger <br /> spills,we can contact Morgan Environmental Service,Hazardous Waste Management, <br /> 1233 21st Street,Oakland, CA 94607, (510)267-0134 phone/(510) 533-2001 24-hour <br /> phone. <br /> Equipment failures that can be handled in-house will be handled by Gambro Healthcare <br /> Technical Services. An outside company will handle any other equipment failures. <br />
