HSC 117960 - LQG shall file MWMP with the EA <br />OBSERVATION: The Medical Waste Management Plan (MWMP) is inadequate. The plan was not certified and did not include <br />an emergency action plan. <br />CORRECTIVE ACTION: Update the MWMP to include an emergency action plan, certification statement, and all items listed in <br />HSC 117960. Submit a copy of the revised plan to the Environmental Health Department (EHD). <br /> <br />#23. HSC 117630 - Biohazard bag shall meet the standards set forth in this section. <br />OBSERVATION: The red biohazardous waste bags observed on-site are not compliant with ASTM D1709. The bags on-site <br />were printed with the following label “MEETS REQUIREMENTS AS FOLLOWS: ASTM: D17069-01 165gram Dart Impact, <br />ASTM: D1922-00a 480grams Elmendorf Tear”. <br />CORRECTIVE ACTION: Per HSC 117630, “a biohazardous bag that is used to collect medical waste within a facility shall be <br />manufacturer certified to meet the ASTM D1709 dart drop test, provided that when the bag is prepared for transport offsite, it is <br />placed into a USDOT-approved container lined with a biohazard bag that is ASTM D1709 and ASTM D1922 certified”. Ensure all <br />bags used on site are compliant with HSC 117630. Send photos of the new bags to the EHD. <br /> <br />#25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br />OBSERVATION: <br />1. The container for the soiled linen towels (labeled as biohazardous) is inadequate. The bag does not properly line the <br />container. The bag was cut down one side of the bag to make it fit into the container. <br />2. Biohazardous waste stored above 32 Fahrenheit is stored on site more than 7 days. Per Lori and facility disposal <br />records, waste is picked up every 2 weeks. <br />CORRECTIVE ACTION: <br />1. Provide a closed rigid container that can be properly lined. Send photos of the new container to the EHD. <br />2. Ensure that all biohazardous waste stored above 32F is not stored on-site for more than 7 days. Biohazardous waste <br />stored onsite at or below 32F may not be stored more than 90 days. Provide a statement explaining how the facility will <br />ensure compliance moving forward. <br /> <br />Submit evidence of corrections to the EHD by: <br />05/21/2025. <br /> <br />Notes: <br />1. The inspection report was emailed to Lori Vukmirovich lvukmirovich@pacmedical.com. <br />2. Reviewed their MWMP and disposal records. <br />3. Medical waste hauler is ATI Medical Waste Management. <br />4. Pathological waste are kept in freezers and picked up every other Thursday. Per Lori, moving forward, biohazardous <br />waste will also be stored in the freezers until pick-up.
