Biohazard bag does not meet the standard set forth in HSC 117630. - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 The biohazard waste bags in use in the facility do not meet both the ASTM D1709 and the ASTM D1922. Biohazard bags used to ship waste off site must meet both standards. Submit evidence of correction to the EHD by 3/13/2019. 24. Medical waste is not segregated at point of generation. -Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a) (1) Pharmaceutical and biohazardous waste was discarded in a sharps container. Submit evidence of in-service training on the segregation of biohazardous, pharmaceutical,and sharps waste to the EHD by 3/13/2019. 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words "Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(4) The chemotherapy waste container in the pharmacy area is not labeled on all lateral sides. Submit evidence of correction to the EHD by 3/13/2019.
