Biohazard bag does not meet the standard set forth in HSC 117630. <br /> - Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br /> The biohazard waste bags in use in the facility do not meet both the ASTM D1709 and the ASTM D1922. Biohazard <br /> bags used to ship waste off site must meet both standards. Submit evidence of correction to the EHD by 3/13/2019. <br /> 24. Medical waste is not segregated at point of generation. <br /> -Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a) (1) <br /> Pharmaceutical and biohazardous waste was discarded in a sharps container. Submit evidence of in-service training <br /> on the segregation of biohazardous, pharmaceutical,and sharps waste to the EHD by 3/13/2019. <br /> 24. Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled with the words <br /> "Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(4) <br /> The chemotherapy waste container in the pharmacy area is not labeled on all lateral sides. Submit evidence of <br /> correction to the EHD by 3/13/2019. <br /> All corrections are due by 3/13/2019. <br /> Notes.- <br /> 1) <br /> otes:1) Barnett Medical Services is the licensed medical waste hauler for the facility. Medical waste in the form of biohazardous <br /> and chemotherapy waste is picked-up once per week. <br /> 2) The Medical Waste Management Plan is current and available along with annual training records. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
