Trace chemotherapy waste is not segregated for storage. The secondary container is not labeled appropriately. <br /> -Trace chemotherapy waste shall be segregated for storage, and the secondary container shall be labeled V <br /> "Chemotherapy Waste"or"CHEMO"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(x)(4) <br /> The chemotherapy waste container in the pharmacy area is not labeled on all lateral sides. Submit evidence of <br /> correction to the EHD by 3/17/2020. <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - 'oitr� oactive and non-RC Armaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY"on the lid and sides, so as to be visible <br /> from any lateral direction. HSC 118275(a)(6)(B). Pharmaceutical waste containers are not labeled "INCINERATION ONLY" <br /> or"HIGH HEAT"on the lid and on all four sides. Submit evidence of correction to the EHD by 3/17/20. <br /> All corrections are due by 3/17/2020. <br /> Evidence of correction can be submitted to the EHD at: ieaster(a�sigov.org <br /> Failure to submit corrections by 3/17/2020 will result in a re-inspection charged at the EHUs hourly rate of$152 per hour. <br /> Notes: <br /> 1) Medical waste in the form of biozardous, chemotherapy, and pharmaceutical waste is picked-up once per week by <br /> Barnett Medical Services. <br /> 2) The Medical Waste Management Plan is current and available along with annual training records. <br /> 3) The biohazard waste bags in use in the facility meet the ASTM D-1709 and can only be used to collect medical waste <br /> within the facility. This bag must be placed in a bag that meets both the ASTM D-1709 and ASTM D-1922 standards <br /> prior to shipping waste offsite. The best practice for biohazard waste bags would be to only use bags that meet both the <br /> ASTM D-1709 and ASTM D-1922 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
