(EPA# <br /> CAL000331285;Transporter ID#4891). A medical waste inspection was done at Stockton Hematology-Oncology(2626 <br /> N. California St., Stockton,CA 95204)on 4/1/14. It was requested that the facility contact Barnett Medical Services and <br /> obtain the treatment records for their medical waste and then submit copies to me. On 4/14/14, Stockton Hematology- <br /> Oncology emailed me the treatment records. <br /> Upon review of the treatment records, it was discovered that four records(4/2/13,4/9/13,4/16/13,and 4/23/13)had a <br /> signature and date certifying destruction via autoclave that was before the date of the actual pickup. For example,the <br /> 4/2/13 manifest(the medical waste was picked up on 4/2/13)indicates that the medical waste was treated via autoclave on <br /> 3/4/13. Attached are the four treatment records.
