Field Operations Policy and Procedures Preparation Room Cleaning and Decontamination Requirements Form The Preparation Room Cleaning and Decontamination Requirements Form (FIN-CS102) is specific to each individual preparation room to communicate cleaning and decontamination requirements. Preparation Room Cleaning and Decontamination Schedule Form The Preparation Room Cleaning and Decontamination Schedule Form (FIN-CS101) must be used to document cleaning and decontamination of a preparation room. Refrigeration Cleaning Log The Refrigeration Cleaning Loy(FIN-CS100) must be kept in an easily accessible area near each refrigeration unit and is to be used whenever each unit is cleaned and sanitized. Refrigeration Log Post the Refrigeration Log (FIN-CS085) on or near the refrigeration unit so that it is easily accessible. Use it to log in each decedent upon initial arrival in the refrigeration unit and to log them out when they are transferred to other areas within the location (i.e. Prep Room, Embalming Room) or to another facility. The log must also be used to keep track of decedents transferring in or out of the refrigeration unit due to overflow conditions. Refrigeration Overflow Log The Refrigeration Overflow Log (FIN-CS086) must posted on or near each refrigeration unit at a location. It is to be used any time a decedent is placed in or removed from a refrigeration unit due to an overflow situation. Refrigeration Overflow Plan Post the Refrigeration Overflow Plan (FIN-CS098) on the primary refrigeration unit so that it is easily accessible. The plan is used to designate alternate refrigeration locations for storage as well as to designate who must be contacted in case an overflow situation occurs. Release Authorization The Release Authorization Form (FIN-CS027) is used to authorize the release of a decedent from the place of death or a funeral home into the custody of a Company funeral home or a non- Company institution. The form must be signed by the Authorized Representative. Safe Log A log at each location on which to keep track of when decedent valuables are placed into or removed from a designated locked, secure area. Service Agreement The Service Agreement is a contract between Company affiliated location(s) and third party vendor(s)who provides services on behalf of the Company affiliated location(s).
