This registered large quantity generator does not file a medical waste management plan (MWMP) <br /> with San Joaquin County Environmental Health Department(EHD). <br /> -A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms <br /> prescribed by the EHD. HSC117960. Any corrections to the Medical Waste Management plan are to be submitted to the <br /> EHD (This violation was corrected during the inspection). <br /> 24. Consolidated waste is not treated by incineration. The container of consolidated waste is not <br /> labeled appropriately. <br /> -Consolidated waste shall be treated by incineration, and the container shall be labeled with the <br /> biohazardous waste symbol and the words"HIGH HEAT OR INCINERATION ONLY" on the lid and side so <br /> as to be visible from any lateral direction. HSC 118275(a)(7). The pharmaceutical waste is stored in a box that is not <br /> properly labeled. Pharmaceutical waste is to be stored in a container which is marked "High Heat or Incineration Only" <br /> on the lid and all sides. The container should be labeled with the date it is first put in use. Submit evidence of <br /> correction the EHD by 2/14/19. <br /> All corrections are due by 2/14/19. <br /> Notes: <br /> 1) The facility is using Stericycle as a licensed medical waste hauler with weekly pick-ups. <br /> 2) Pharmalink is used to dispose of unused and expired pharmaceutical waste. Records are available for at least one pick- <br /> up per year for the last two years. <br /> 3) There was one Stericycle pick-up of pharmaceutical waste on 9/6/17. <br /> 4) The Medical Waste Management Plan meets the requirements, and training records are available for all employees <br /> (approximately 350). <br /> 5) The biohazardous waste bags in use meet both the ASTM D1709 and D1922 standards. <br /> Reinspecion oniabout: A reinspection fee of$152 per hour may be charged.
