Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste <br /> management plan. Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6) <br /> -Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be <br /> placed in a container or secondary container labeled with the words"HIGH HEAT OR INCINERATION <br /> ONLY"on the lid and sides, so as to be visible from any lateral direction. HSC 118275(a)(6)(B) The Pharmaceutical waste <br /> containers are labeled "incineration only"on the lid and two sides. The containers must be labeled on the top and all <br /> four lateral sides. Submit evidence of correction by 1/23/19. <br /> Notes: <br /> 1) Stericycle is the registered hauler for biohazardous waste, and pick-ups are done every 7 days. The frequency of pick- <br /> ups is expected to increase to twice a week as they are planning on increasing the number of patients per week. <br /> 2) The medical waste management plan meets the requirements of HSC117960. <br /> 3) There are 17 employees. Employee training is conducted annually. The last training was conducted on 1/26/17. <br /> 4) The biohazardous waste bags meet both the ASTM D1709 and D1922. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
