HSC 117960 - LQG shall file MWMP with the EA. <br />OBSERVATION: The Medical Waste Management Plan (MWMP) is missing an emergency action plan. Per the Emergency Action Plan <br />section on page 8, “A contingency plan for disruption of medical waste disposal service is required. This must be addressed in the policy: <br />Disaster and Emergency Plan. All facilities are to house their plan as directed by the Disaster and Emergency Plan policy. Fa cility <br />Emergency Management Plan (EMP) shall be maintained”. Upon review of the facility’s Emergency Management Plan on site and per <br />discussion with site personal, their EMP does not include a contingency plan for disruption of medical waste disposal service . <br /> <br />CORRECTIVE ACTION: Submit to the Environmental Health Department a MWMP that includes all required elements listed in HSC <br />117960. <br /> <br />#24. HSC 118275 - Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and <br />contained pursuant to this section. The container shall be properly labeled. <br />1. OBSERVATION: Multiple biohazardous containers were missing proper labeling on the back of the containers. Labelling was <br />corrected on site at the time of the inspection. <br />CORRECTIVE ACTION: Provide training to all staff regarding property labeling of containers and submit training records to the EHD. <br /> <br />2. OBSERVATION: The pharmaceutical waste container was missing proper labeling on 3 sides of the container. This was corrected on <br />site. <br />CORRECTIVE ACTION: Provide training to all staff regarding property labeling of containers and submit training records to the EHD. <br /> <br />3. OBSERVATION: A sharps container was observed with gloves inside. <br />CORRECTIVE ACTION: Ensure that all medical waste are contained separately from other waste at the point of origin. Provide <br />training to all staff regarding waste segregation and proper waste disposal and storage. Submit training records to the EHD. <br /> <br />#25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br />OBSERVATION: Containers near Pod 2 and Pod 4 need repair or replacement. The containers were missing one of the metal rods that <br />hold the liner in place. Due to the missing component, not all waste is going inside the liner. <br />CORRECTIVE ACTION: Repair or replace the containers to ensure containers are “leak resisteant, have tight -fitting covers, and kept <br />clean and in good repair”. Submit photos to the EHD. <br /> <br />Correct all violations and submit evidence to the EHD by <br />March 13, 2025. <br />Notes: <br />1. Inspection report and photos were emailed on 2/18/25 to Narey Khun (Facility Administrator) at Narey.khun@davita.com <br />2. Biohazardous bags meet ASTM D1709 & ASTM D1922 standards. <br />3. Medical waste hauler: Stericycle. Waste is picked up every 2 -3 days. <br />4. Facility generates pharmaceutical waste, sharps, and biohazardous waste. <br />5. Tracking records were reviewed from 02/01/2023-02/12/2025. <br />6. Deisgnated accumulation area and treatment floor inspected.
