The container was not labeled <br /> with the date the container was put in use. (This violation was corrected during the inspection) <br /> 19. For steam sterilization, the temperature does not reach 250 F for 30 minutes. Thermometer is not calibrated annually. <br /> - Recording or indicating thermometers shall be checked during each complete cycle to ensure the attainment of 250 F for at <br /> least 30 minutes. HSC118215(2)(B) <br /> -Thermometers, thermocouples, or other monitoring devices shall be checked for calibration annually. Records of the calibration <br /> checks shall be maintained as part of the facility's files and records for 2 years. HSC 118215(a)(2)(B). Calibrate the autoclave <br /> thermometer and submit evidence of calibration to the EHD by 3/8/19. Autoclave must not be used until calibration is <br /> done. This is a repeat violation. <br /> Notes: <br /> 1) The Medical Waste Management Plan was available for review along with training records for three employees for 2018, <br /> and two employees for 2017 and 2016. <br /> 2) Sharps Compliance's Yellow Dot mail-back program is used for sharps disposal and pharmaceutical waste disposal. <br /> The waste is consolidated in a single container with a biohazardous waste symbol and "Incinerate Only" on all sides and <br /> the lid (Corrected during inspection). <br /> 3) Treatment records are available for review from 5/12 to current. <br /> 4) The biohazardous waste bags in use meet both the ASTM D1709 and D1922 standards. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
