N <br /> Life Line Scrtni <br /> Limited Quantity Hauling Exemption <br /> Need Statement <br /> Life Line Screening operates mobile vans throughout California with its corporate <br /> headquarters located in Cleveland, Ohio. Our Ultrasound and Medical <br /> Technicians provide screenings for vascular disease and osteoporosis. <br /> Screenings are held in public places such as churches, community centers or <br /> recreational centers and locations will vary from day to day. <br /> In addition to providing ultrasound vascular screenings, each team will also be <br /> offering finger stick blood tests for Lipid Panel and Glucose. Each mobile van will <br /> have in their possession one or two 3 gallon sharps containers for the disposal of <br /> single use retractable lancets, pipette tips, glass vials of quality control material, <br /> and soft goods saturated with blood. <br /> Life Line Screening will require a Limited uantity Hauling Exemption for <br /> the legal transport of the biohazard sharps containers from site to site until <br /> the containers become filled. <br /> For the proper disposal of full containers, Life Line Screening has contracted with <br /> an approved mail-back sharps provider, Sharps Compliance, Inc. Managers of <br /> each team are instructed to mail back containers when they reach 3/ full in the <br /> regulated shipping cartons provided by the manufacturer. Each container is <br /> accompanied by a tracking document that acts as a manifest and guarantees the <br /> proper disposal of the filled container by Sharps Compliance. The original <br /> document accompanies the container to be disposed of and a copy is retained by <br /> the team manager. <br />
