d <br /> 3. The application for an authority to construct or <br /> permit to operate shall include a detailed description <br /> of the wastes approved for disposal at the site, the <br /> volume of waste received or expected to be received, <br /> the expected emissions associated with the operation of <br /> the facility, and the approved size ( in acres) of the <br /> facility. In addition to the above, all applications <br /> shall include such other information, data, or drawing <br /> as necessary to render the application complete as <br /> required by the rules and regulations of the District. <br /> Applications for all such permits shall be made on <br /> forms provided by the Air Pollution Control Officer <br /> (APCO) . <br /> At the time the authority to construct or permit to <br /> operate application is filed with the APCO, the <br /> affected facility shall submit a plan. The plan shall <br /> at least one of the following: <br /> plot <br /> a. Methods used to monitor odors. <br /> b. Precautions to be taken to prevent creation of <br /> nuisance odors. <br /> c . Procedures to deal with public complaints. <br /> d. The names, titles, responsibilities, and telephone <br /> numbers of responsible persons who, one of whom, <br /> may be contacted at all times while the facility <br /> is in use, and <br /> e . If applicable, the information required by Section <br /> 25155. 10 of the California Health and Safety Code <br /> relating to ambient air monitoring plans. <br /> f. Precautions to be taken to prevent the mixing of <br /> incompatible wastes. <br /> If the APCO determines the facility is so designed, <br /> control, or equipped that it may be expected to <br /> comply with the applicable rules, a permit shall be <br /> issued as provided by Rule 207 . <br /> 4 . Not later than 30 days prior to the expiration of <br /> the permit to operate , the affected facility shall <br /> submit to the APCO the data necessary to document <br /> compliance with the permit to operate and the <br /> applicable rules and regulations of the District. <br /> The APCO shall review this data and other available <br /> data to determine compliance . If the facility is <br /> found to be in compliance, the APCO shall re-issue <br /> the permit to operate. <br />
