ATTACHMENT "C" <br />DECLARATION <br />r " <br />Before The: STATE SOLID WASTE <br />MANAGEMENT BOARD <br />Solid Waste Facility Permit <br />No. 39 -AA -001 <br />Declaration of Exemption From <br />Surcharge Pursuant to California <br />Government Code Section 66796.22 <br />I, the undersigned, say: <br />1. I am the operator of the following named solid waste <br />operator/owner <br />disposal site: Austin Road landfill site <br />Address: 1465 South Lincoln <br />City, County: Stockton, San. Joaquin Zip: 95206 <br />Telephone: 944-8420 <br />2. I hereby certify that I am eligible,for an exemption from the surcharge imposed <br />by Government Code section 66796.22 because I have satisfied and will continue <br />to satisfy at least one of the requirements of subdivision (c) of section 66796.22, <br />as indicated: <br />XX a. (All) (Part of) the solid wastes disposed of at the above-mentioned <br />solid waste disposal site cannot be practically or economically disposed <br />of other than by burial at a solid waste disposal site. (Government <br />Code section 66796.22 (c) (1) ) <br />Comments: <br />b. (All) (Part of) the solid wastes disposed of at the above .named disposal <br />site are disposed of by persons who engage in practical and economical <br />separation prior to final disposal in a manner which minimizes the amount <br />of wastes disposed of by burial. (Government Code section 66796.22 (c) <br />(2) ) <br />Comments <br />3. I will inform the Executive Officer of the State Solid Waste Management Board in <br />the event the conditions qualifying the above exemption change. I understand that <br />if I no longer am satisfying the requirements of Government Code section 66796.22 <br />(c) I shall be required to pay the surcharge. <br />4. I certify that I will make available upon request during normal work hours all <br />records which document and authenticate any exemption granted under provision 2. <br />5. On January 19 , 1979 , declarant requests from the State Solid Waste Management <br />Board an exemption from surcharge pursuant to Government Code section 66796.22. <br />6.
