Li <br />• Date and time of collection <br />• Type of preservative used (if any) <br />Sampling and Analysis Chain -of -Custody Record <br />The sampling and analysis chain -of -custody record (see Figure A-3) initiated at the time of <br />sampling contains, but is not limited to, the well number, sample type, analytical request, date of <br />sampling, and the name of the sampler. The record sheet will be signed, timed, and dated by the <br />sampler when transferring the samples. Custody transfers will be recorded for each individual <br />sample; for example, if samples are split and sent to more than one laboratory, a chain -of -custody <br />record sheet will accompany each sample. The number of custodians in the chain of possession will <br />be kept to a minimum. A copy of the sampling and analysis chain -of -custody record is returned to <br />EMCON with the analytical results. <br />Groundwater Sampling and Analysis Request Form <br />A groundwater sampling and analysis request form (see Figure A-4) or purchase order, or both, will <br />accompany the samples delivered to each laboratory. These forms serve as official communication <br />to the laboratory of the particular analysis/analyses required for each sample. <br />At minimum, the groundwater sampling and analysis request form includes the following <br />information: <br />• Date submitted <br />• Specific analytical parameters <br />• Well number <br />' • Sample source <br />' ANALYTICAL METHODS <br />Samples collected as part of the monitoring program will be analyzed consistent with accepted <br />' analytical procedures.
