• Date and time of collection <br /> • Type of preservative used(if any) <br /> Sampling and Analysis Chain-of-Custody Record <br /> The sampling and analysis chain-of-custody record (see Figure A-3) initiated at the time of <br /> sampling contains, but is not limited to, the well number, sample type, analytical request, date of <br /> sampling, and the name of the sampler. The record sheet will be signed, timed, and dated by the <br /> sampler when transferring the samples. Custody transfers will be recorded for each individual <br /> sample; for example, if samples are split and sent to more than one laboratory, a chain-of-custody <br /> record sheet will accompany each sample. The number of custodians in the chain of possession will <br /> be kept to a minimum. A copy of the sampling and analysis chain-of-custody record is returned to <br /> EMCON with the analytical results. <br /> Groundwater Sampling and Analysis Request Form <br /> A groundwater sampling and analysis request form(see Figure A-4) or purchase order, or both, will <br /> accompany the samples delivered to each laboratory. These forms serve as official communication <br /> to the laboratory of the particular analysis/analyses required for each sample. <br /> At minimum, the groundwater sampling and analysis request form includes the following <br /> information: <br /> • Date submitted <br /> • Specific analytical parameters <br /> • Well number <br /> • Sample source <br /> ANALYTICAL METHODS <br /> Samples collected as part of the monitoring program will be analyzed consistent with accepted <br /> analytical procedures.
