U"&IAccident/Incident TerminoloQy <br /> Tier 1 Minor Incident <br /> An event that causes or had potential to cause only minor damage or injury requiring only first aid treatment. <br /> • A small fire not requiring assistance to extinguish. <br /> • Near misses where injury or damages could have occurred, but are limited to first-aid injuries and <br /> property damage of less than$5,000. <br /> • Motor Vehicle Accident with damage less than$5,000 <br /> • Fall into water or resulting in deployment of lanyard. <br /> • Environmental spill less than 25 gallons; <br /> o That does not violate any reportable quantity <br /> o And does not contact a navigable waterway. <br /> Tier II Serious Incident <br /> Any event that required or had the potential to require emergency services for treatment of an injury or control <br /> of hazards to personnel or the environment to include: <br /> • Recordable injuries/illnesses <br /> • Restricted workday injuries/illnesses <br /> • Any incident that causes damage to equipment requiring extensive repair or causes a disruption in <br /> work progress <br /> • Near misses where injury or damages could have occurred, but are limited to recordable injuries, <br /> restricted work injuries,or property damage of$5,000 to$50,000 <br /> • Serious environmental consequences(any quantity of release to the environment requiring reporting to <br /> relevant governmental agencies). <br /> • Reputation or community relations affected <br /> • Environmental spill of 25 to 1,000 gallons, <br /> o That does not contact a navigable waterway <br /> o but is in violation of a reportable quantity <br /> • High potential incidents as defined above <br /> • Could affect reputation or community relations <br /> Tier III Major Incident <br /> Any event causing or having the potential to cause multiple injured/ill personnel requiring emergency services, <br /> permanently disabling injuries/illness, fatalities, property damage in excess of $50,000, disruption of critical <br /> plant systems,or environmental impacts requiring regulatory notifications to include: <br /> • Injuries/illnesses that cause lost work time <br /> • Fatality <br /> • Hospital admittance(excluding emergency room outpatient care) <br /> • Major fire or chemical spill,greater than 1,000 gallons,or a spill of any amount that contacts a <br /> navigable waterway, <br /> • Radiation exposure <br /> • Lost radioisotope <br /> • Bomb threat,weapons discharge,terrorist act, NGO confrontation <br /> • ISOS evacuation <br /> • Any injury or illness meeting the definition of lost time <br /> • Any incident causing damage to equipment or resources that will disrupt work activities <br />
