Although this HASP contains the safety requirements for the identified work tasks, this process is critical <br /> to identifying changes in the hazard scenario or identifying new hazards that need to be addressed. If there <br /> are any questions regarding this process or assistance is required, contact the local HSM. <br /> 5X5 Analysis/STARRT Cards <br /> An additional step in the Work Hazard Analysis/Recognition Process (CMS -710-05-PR-01700) is to <br /> develop a 5 X 5 Analysis/Safety Task Analysis and Risk Reduction Talk (STARRT) Card or equivalent <br /> form) just prior to the performance of the work. The job or task specific analysis is performed by the <br /> individual who is to perform the work. This is the individual's opportunity to confirm that he has <br /> everything needed to perform the work in a safe manner. <br /> The individual will prepare job step specific analysis,using one of the following found in Appendix A: <br /> • CMS-710-05-FM-01706—Sample 5x5 Analysis <br /> • CMS-710-05-FM-01707—Sample STARRT Card <br /> The work crew will be actively involved in conducting a 360° review of their specific work area, <br /> identifying hazards of their work and the hazards of work activities surround them. The work crew lead <br /> and the work crew will also collectively review the means of mitigation and ensure proper controls and <br /> measures are in place. <br /> The job or task specific analysis is a tool for the individual to identify any unaccounted hazards that one <br /> may encounter associated with the tasks he will actually be performing. <br /> 1.3.5 Injury Management and Hospital Route Maps <br /> Injury Management/Incident Notification <br /> Observe the following injury management/incident notification procedures and practices: <br /> Injury Management <br /> Observe the following injury management procedures and practices: <br /> • Once a personal injury incident is discovered the first action will be to ensure the injured party <br /> received appropriate medical attention. <br /> • If it is safe to do so, the nearest workers will immediately assist a person who shows signs of <br /> medical distress or who is involved in an accident. <br /> • The work crew supervisor will be summoned. The work crew supervisor will immediately make <br /> contact with the PM or other designated individual to alert them of the medical emergency. The <br /> work crew supervisor will advise and perform the following"Care of the Employee": <br /> - Location of the victim at the work site. <br /> - Nature of the emergency. <br /> - Whether the victim is conscious. <br /> - Specific conditions contributing to the injury, if known. <br /> - Escort the injured person to the occupational clinic or hospital or arrange for ambulance. <br /> - Contact CORE (877-347-7429) to inform them of the clinic or hospital the employee is going to <br /> be treated and to arrange for a drug and alcohol test <br /> Notification Requirements <br /> Directly After"Care of the Employee,"make the following notifications, in order: <br /> 1-8 <br />
