If there <br /> are any questions regarding this process or assistance is required, contact the local HSM. <br /> 5X5 Analysis/STARRT Cards <br /> An additional step in the Work Hazard Analysis/Recognition Process (CMS -710-05-PR-01700) is to <br /> develop a 5 X 5 Analysis/Safety Task Analysis and Risk Reduction Talk (STARRT) Card or equivalent <br /> form) just prior to the performance of the work. The job or task specific analysis is performed by the <br /> individual who is to perform the work.
