If there are any questions regarding this process or assistance is required, contact the local HSM. 5X5 Analysis/STARRT Cards An additional step in the Work Hazard Analysis/Recognition Process (CMS -710-05-PR-01700) is to develop a 5 X 5 Analysis/Safety Task Analysis and Risk Reduction Talk (STARRT) Card or equivalent form) just prior to the performance of the work. The job or task specific analysis is performed by the individual who is to perform the work. This is the individual's opportunity to confirm that he has everything needed to perform the work in a safe manner.
