In <br /> addition to the completed SWFP application form,this submittal also includes two(2) <br /> copies of the following items: <br /> 1. Most current documentation showing an acceptable closure/post-closure <br /> financial assurance mechanism (27 CCR,Section 21570(f)(7))and compliance <br /> with operating liability requirements(27 CCR,Section 21570(f)(8)); <br /> 2. Amended pages of the Joint Technical Document(JTD) including an updated <br /> cover and spine for Volume 1, revised pages 4-11 and 4-12,Table 8, and <br /> Appendix C - Site Capacity Calculations to reflect an updated estimate of the <br /> remaining site capacity/site life, and amended Appendix L - Preliminary <br /> Closure/Post-Closure Maintenance Plan (PCPCMP)(discussed below) in <br /> accordance with 27 CCR,Sections 21640(2)and (3)(insert these amended <br /> pages to your current copy of the JTD); and, <br /> 3. Updated Survey Capacity Results provided on CD-ROM in accordance with 27 <br /> CCR,Section 21570(f)(9). <br /> The amended portions of the stand-alone PCPCMP(also included as Appendix L of the <br /> JTD)will be submitted shortly to the regulatory agencies under separate cover. <br /> Amendments to the PCPCMP will include an updated cover and spine, revised text,Table <br /> 2 (Diminishing Landfill Capacity),Table 8(Closure Cost Estimate Summary), and Table 9 <br /> (Annual Post-Closure Maintenance and Monitoring Cost Estimate Summary). The <br /> amended PCPCMP will be submitted in accordance with 27 CCR,Section 21640(4). <br /> 1145 W.
