There are no changes in <br />design or operation of the facility. Changes in the terms and conditions of the permit <br />are not necessary. During the 5 year permit review process sections of the JTD were <br />amended to comply with Title 27 California Code of Regulation (T27CCR), Sections <br />21815 and 21820. Those changes occur in the Preliminary Closure Post Closure <br />Maintenance Plan (Appendix L). Appendix A of the JTD was amended to include <br />updated Waste Discharge Requirements and Air Pollution Control District permits. <br />Enclosed are the permit application form and Amendment #4, as well as, the 5 year <br />permit review form, Report of Facility Information (RFI) amendment check list for <br />Amendments# 3 & #4, a permit cover page with the new 5 year permit review date. <br />Amendment #3 was rejected due to complication with the Partial Final Closure Plan for <br />Stage IA. The Solid Waste Facility Permit form indicates that Amendment #3 was <br />submitted, but the form indicates it was accepted on August 20, 2008. To clarify <br />Amendment #4 was accepted and Amendment #3 was rejected. <br />
