Violation T27CCR 21600 — Report of Disposal Site Information — The JTD has not been amended to include the information <br />listed in T27CCR2I600(bx6)(B), the ADC usage and waste to cover ratin,,,. <br />a. Submit application and RDSI amendment to include all information listed in T27CCR216000 (b)(6)(B) to LEA for <br />approval. <br />b. Also see inspection dated 4/20/05,5/25/05,6/24/05,7/27/05,8/26/05,9/29/05, 10/28/05, 11/28/05,12/28/05, <br />1/24/06, 2/08/06, 3/30/06, 4/21/06, 5/18/06, 6/23/06, 7/28/06, 8/31/06, 9/29/06, 10/27/06 and 11/29/06 for previous <br />violation of this section. <br />c. The proposed JTD amendment rcccivcd on August 2nd, 2005 was rejected by this LEA in a response letter dated <br />August 23d, 2005 due to conflicting with existing approvals for the facility. <br />d. The proposed JTD Amendment received on January 1, 2006 was rejected by the LEA in a response letter dated <br />March 17, 2006 due to noncompliance with Title 27 CCR Section 21665. A meeting was held with Forward Inc. on <br />March 23, 2006 regarding this issue. <br />e. The proposed JTD Amendment received on May 23, 2006 was rejected by this LEA in a response letter dated Jane <br />22, 2006 because it did not meet Title 27 CCR Section 21665. <br />f, On October l I and 12, Forward Inc., CIWMB staff and EHD staff conducted an onsite ADC volume/density test. <br />The draft report of the testing was received by the EHD on November 11, 2006. The results will be, used to <br />determine allowable site usage of ADC for the JTD amendment. <br />g. To date, the EHD has not received any additional information regarding the JTD submittal. <br />2. Violation T27CCR 20580 — Lighting — Inadequate lighting: tarps were removed in the dark, the portable lights were so far <br />away. On the class 11(C11) site, the TASW was dumped in the dark; the portable light was so for from the dumping <br />area. One portable light was provided at the Cil area, One portable light and the tipper light were provided at the <br />CIII area. One portable light is not in use. Drivers removed tarps or cleaned their trucks in complete darkness. <br />a. Provide adequate lighting. <br />b. Ensure that all lights are functional. <br />3. Violation T27CCR 20590 — Personnel .Health & Safety — Forward employees removed the tarps at CIII area without hardhat, <br />Several -drivers were walking without hardhats or safety vests in the dumping areas. <br />a. Provide adequate personnel safety equipment. <br />b.
