WASTE DISCHARGE REQUIREMENTS ORDER NO.R5-2003-0049 -30- <br /> FORWARD INC. AND ALLIED WASTE INDUSTRIES <br /> FOR OPERATION OF <br /> FORWARD CLASS II LANDFLLL <br /> SAN JOAQUIN COUNTY <br /> f. An evaluation of the effectiveness of the leachate monitoring/control facilities. <br /> 7. The Discharger or persons employed by the Discharger shall comply with all notice and <br /> reporting requirements of the State Department of Water Resotuces with regard to the <br /> construction, alteration, destruction, or abandonment of all monitoring wells used for <br /> compliance with this Order or with MRP No.R5-2003-0049, as required by Section <br /> 13750 through 13755 of the California Water Code. <br /> 8. The Discharger shat I noti fy the board in writing of any proposed change in ownership or <br /> responsibility for construction or-operation of the landfill. The Discharger shall notify the <br /> succeeding owner or operator in writing of the existence of this Order. A copy of that <br /> notification shall be sent to the Board. <br /> 9_ The Board will review this Order periodically and will revise these requirements when <br /> necessary. <br /> H. PROVISIONS <br /> 1. The Discharger shall maintain a copy of this Order at the facility and make it available at <br /> all times to facility operating personnel, who shall be familiar with its contents, and to <br /> regulatory agency personnel. <br /> 2. The Discharger shall comply with all applicable provisions of"Title 27 and 40 Code of <br /> Federal Regulations Part 258 (Subtitle D) that are not specifically referred to in this <br /> Order. <br /> 3. The Discharger shall comply with M.oni.tonng and Reporting Program No. <br /> R5-2003-0049, which is incorporated into and made part of this Order. <br /> 4. The Discharger shall comply with the applicable portions of the Standard Provisions and <br /> Reporting Requirements for Waste Discharge Requirements for Nonhazardous Solid <br /> Waste Discharges Regulated by Title 27 and/or Subtitle D (27 CCR §20005 et seq, and <br /> 40 ('FR 258 et sect..), dated April 2000,which are hereby incorporated into this Order. <br /> 5. All reports and transmittal letters shall be signed by persons identified below: <br /> a. For a corporation: by a principal executive officer of at least the level.of senior <br /> vi ce-pr.esident. <br /> b. For a partnership or sole proprietorship: by a general partner or the proprietor. <br /> c.
