Violation T27CCR 20919.5—Explosive Gas—The facility has explosive gas at the boundary above 5%.The following <br /> perimeter gas wells were monitored.The following was noted. <br /> GP7(S) .2%CH4 GP8(S)0.0%CH4 GP9(S)4.2%CH4 GP11 (S) 15.0%CH4 GP12(S).16.9%CH4 <br /> (1) .0%CH4 (I)0.0%CH4 (I)0.0%CH4 0 0 <br /> (D) .0%CH4 (D)0.0%CH4 0 (I) 15.7 /o CH4 (I) 10.5 /o CH4 <br /> (D),0.0/o CH4 (D)5.7% CH4 (D)2,7% CH4 <br /> The facility operator needs to take the necessary steps to control landfill gas at the boundary.Any changes or modifications <br /> to the gas extraction system must have:prior approval from the SJVAQMD and the EHD. Review the Operation and <br /> Maintenance(O&M)manual to determine if all require maintenance was conducted.Evaluate the O&M manual to determine <br /> if maintenance,is adequate. <br /> 2. AC T27CCR20650—Gradin of F`il — <br /> g 1 Surfaces Ponding at top of the deck ofold-Austin Road. <br /> a. Regrade the fill surface to'promote lateral run off. <br /> b. Ensure no ponding occurs; <br /> 3, AC T27CCR20860—Traffic Control--At tipping area,the traffic did not have clear pattern or organization.The site <br /> personnel were not able to control traffic.The tipping area was confined to a small area. <br /> a. Provide adequate traffic ctintrol. <br /> 4. AC T27CCR 20590—Personnel Health&Safety—Forward employees working around the tipping area did not use lighted <br /> beacon to make their location known to the truck traffic dumping at the working face.The site personnel barely <br /> visible while working.in low light operations. <br /> a. Provide adequate personnel si%:rety equipment. <br /> b. Ensure that all personnel, including truck drivers,wear safety equipment when working on active face area <br /> 5. AC T27CCR 20580—Lighting—.The portable,lights were placed a great distance from the tipping area.The portable lights <br /> were being used in the same area,whicv provided some light but not enough to help ensure safe operations. <br /> a. Place the light towers close enough to the tipping area to provide the maximum amount of light to the are where the <br /> trucks are being unloaded.. <br /> b.
