the fourth quarter 2008 monitoring period, (see signed"Certification Statement" located inside cover page, after the Table of Contents in this report). In addition to Forward's inspections,the San Joaquin County Environmental Health Department(CEHD)performed monthly inspections at the facility on October 17, November 19, and December 19, 2008. During the October 17, 2008 inspection the CEHD issued a Notice of Violation(NOV) for failure to weight down the alternative daily cover tarps. Forward has corrected the NOV and no other NOVs were noted during the fourth quarter 2008 monitoring period. Completed inspection forms are presented in Appendix A. 1.4 SOLID WASTE MONITORING According to the Forward Landfill Operations Manager, Forward Landfill received a total of 392,174 tons of solid waste during the fourth quarter 2008 (Appendix A). Figure 1-2 shows the approximate location of waste placement during the monitoring period. The following types of waste were received: Non-hazardous solid waste—382,825 tons Asbestos—875 tons Sewage and wastewater treatment sludge—8,474 tons 1.5 LANDFILL GAS MONITORING No perimeter soil-pore gas probes exceeded the regulatory limit of 5 percent during the fourth quarter 2008. In accordance with California Code of Regulations Title 27, Section 20937, Forward continues to install additional landfill gas extraction wells (as needed), and makes regular adjustments to the landfill gas extraction system to optimize efficiency of the landfill gas extraction system. Furthermore, in accordance with CCR Title 27, Section 20937(a)(1), methane monitors are installed in all buildings at the site to protect public health and safety and the environment. During the fourth quarter 2008, no methane was detected in any of the facilities buildings. A copy of the landfill gas monitoring probe data is provided in Appendix A. 2.0 FORWARD UNIT 2.1 WATER QUALITY MONITORING 2.1.1 Sampling and Laboratory Analyses During the fourth quarter 2008, depth-to-water measurements were recorded in the groundwater monitoring wells on November 10 and 11, 2008, and all wells of the groundwater monitoring wells were sampled by ATC Associates (ATC)between November 10 and 13, 2008. Surface water stations were inspected on November 10, 2008 and reported dry. All lysimeters were inspected and a vacuum was set on November 13, 2008.
