the fourth quarter 2008 monitoring period, (see signed"Certification Statement" located <br /> inside cover page, after the Table of Contents in this report). <br /> In addition to Forward's inspections,the San Joaquin County Environmental Health <br /> Department(CEHD)performed monthly inspections at the facility on October 17, <br /> November 19, and December 19, 2008. During the October 17, 2008 inspection the <br /> CEHD issued a Notice of Violation(NOV) for failure to weight down the alternative <br /> daily cover tarps. Forward has corrected the NOV and no other NOVs were noted during <br /> the fourth quarter 2008 monitoring period. Completed inspection forms are presented in <br /> Appendix A. <br /> 1.4 SOLID WASTE MONITORING <br /> According to the Forward Landfill Operations Manager, Forward Landfill received a total <br /> of 392,174 tons of solid waste during the fourth quarter 2008 (Appendix A). Figure 1-2 <br /> shows the approximate location of waste placement during the monitoring period. The <br /> following types of waste were received: <br /> Non-hazardous solid waste—382,825 tons <br /> Asbestos—875 tons <br /> Sewage and wastewater treatment sludge—8,474 tons <br /> 1.5 LANDFILL GAS MONITORING <br /> No perimeter soil-pore gas probes exceeded the regulatory limit of 5 percent during the <br /> fourth quarter 2008. In accordance with California Code of Regulations Title 27, Section <br /> 20937, Forward continues to install additional landfill gas extraction wells (as needed), <br /> and makes regular adjustments to the landfill gas extraction system to optimize efficiency <br /> of the landfill gas extraction system. Furthermore, in accordance with CCR Title 27, <br /> Section 20937(a)(1), methane monitors are installed in all buildings at the site to protect <br /> public health and safety and the environment. During the fourth quarter 2008, no <br /> methane was detected in any of the facilities buildings. A copy of the landfill gas <br /> monitoring probe data is provided in Appendix A. <br /> 2.0 FORWARD UNIT <br /> 2.1 WATER QUALITY MONITORING <br /> 2.1.1 Sampling and Laboratory Analyses <br /> During the fourth quarter 2008, depth-to-water measurements were recorded in the <br /> groundwater monitoring wells on November 10 and 11, 2008, and all wells of the <br /> groundwater monitoring wells were sampled by ATC Associates (ATC)between <br /> November 10 and 13, 2008. Surface water stations were inspected on November 10, <br /> 2008 and reported dry. All lysimeters were inspected and a vacuum was set on <br /> November 13, 2008.
