A Regulations <br /> x PRC 44014(b)-Operator Complies with Terms&Conditions of Permit <br /> Comments: According to the information the operator submitted to the EHD on June 24, 2010, SG-11 is located <br /> approximately 25 feet from the extent of the waste in Forward Landfill Unit FU-04. <br /> 1. Item 15 of the Solid Waste Facility Permit listed Environmental Impact Report(EIR)as one of the documents <br /> that describe and/or restrict the operation of this facility. <br /> 2. According to the EIR#ER-00-2, dated November 2002,there is to be a 100 feet setback between the waste <br /> and the south property boundary where SG11 and SG12 are located. <br /> 3. The placement of waste is not consistent with the EIR and RFI. <br /> 4. Ensure that the construction and operation of the landfill are consistent with the JTD and the EIR. <br /> 5. The operator shall have the distance between the property boundry and the edge of the waste profesionally <br /> surveyed and submit the result to EHD. <br /> x 20610-Training <br /> Comments:An employee, a load checker, walked pass a television,that was completely exposed near the active <br /> face, 4 times without removing it from the waste pile. That television was later pushed into the active face and <br /> buried. <br /> 1. Provide adequate training on load checking and forbidden waste. <br /> 2. Ensure that there is procedure inplace for universal and hazardous waste removal <br /> x 20680- Daily Cover <br /> Comments: Several dead chicken from a few days ago were not covered in the asbestos area. The area where <br /> the chicken were dumped were close to the mid point of the east side of the dump area. <br /> 1. Ensure all waste is convered at the end of operating day. <br /> 2. The waste in the asbestos area shall be covered in accordance with the JTD. <br /> x 20690-Alternative Daily Cover <br /> Comments: The tarps in use had several large tears that allowed waste to expose. <br /> 1. This issue was documented as an area of concern in the June inspection report. <br /> 2. Replace tarps. <br /> 3. Ensure that all waste is completely covered at the end of each operating day. <br /> x 20870-Hazardous Wastes <br /> Comments: An employee, a load checker,walked pass a television,that was completely exposed near the active <br /> face, 4 times without removing it from the waste pile. That television was later pushed into the active face and <br /> buried. A waste tire was mostly buried on the south slope near the current active face. <br /> 1. Provide adequate training on load checking and forbidden waste. <br /> P. Ensure that there is procedure inplace for universal and hazardous waste removal <br /> x 20921 -Gas Monitoring and Control <br /> Comments: The EHD staff used the Lantec Gem 2000 to measure the concentration of explosive gas at the <br /> property boundaries. The concentrations of methane gas by volume in SG-11 were 17.2% in Shallow, 12.2% in <br /> Intermediate and 12.1% in Deep. The concentration of methane gas in SG-9 and SG-12 were 0% by volume. <br /> 1. Ensure that the concentration of the explosive gas is less than 5% by volume. <br /> 2. Within 60 days of dectection (June 3,2010), implement a remediation plan that has been approved by the <br /> EHD and the CalRecycle (formerly COMB). <br /> Inspection Repon`Comments: <br />
