Submitted to: Scott Purdy (GLA) Project #: 2013-AO55 Report No.: 002 Report Date: Friday May 17, 2013 Onsite: 07:00 - 17:30 Hrs Weather: AM: 68 OF Clear PM: 84°F Clear Guinn Construction Activities: • Guinn construction performed clearing work along the existing North Slope tie-in line exposing WMU-FU 8 synthetic liner. • The Contractor completed subgrade preparation along the toe of the west and north slopes; started buttress fill placement using material from cut floor area. • The Contractor performed continuous moisture conditioning and sheepsfoot compaction on the buttress fill. • Personnel: (1) Superintendent, (1) Grade Checker, (6) Operators, (2) Laborers. Equipment: (1) 825 Cat Sheepsfoot compactor, (1) 140H Cat Motor -Grader, (2) Water trucks, (1) Cat Excavator, (3) D6 Cat Dozers, (4) 637 Cat Scrapers, (1) Cat Fork-lift. CQA (GLA) Activities: Field Personnel (1) • CQA personnel performed continuous monitoring and lab and field moisture -density testing of the buttress fill placement. • CQA personnel monitored subgrade preparation and excavation work along the Cell floor. • CQA personnel monitored the clearing work being performed along the North Slope tie-in line exposing the existing synthetic liner. Meetings and Site Discussions: • Met with Ruben (Forward LF Site Manager) and Kevin (Guinn Superintendent) in the morning to discuss planned activity for the work shift. The Contractor would start buttress fill along the WMU-10 and WMU-8 • existing slope upon completion of the CQA subgrade field testing. The Contractor would continue exposing PAGE 1 OF 2
