is <br />Geo-Lo!lk <br />A S S 0 C I A T E S <br />CQA DAILY SUMMARY REPORT #021 <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />Location: Manteca, CA Owner: Republic Services <br />CQA Consultant: Geologic Associates Contractors: Guinn Construction <br />Prepared by: Jose Armenta (GLA) Comanco Environmental Corp. <br />Submitted to: Scott Purdy (GLA) <br />Project #: 2013-AO55 <br />Report No.: 021 Report Date: Friday June 14, 2013 <br />Weather: AM: 59 OF Clear <br />Guinn Construction Activities: <br />Onsite: 06:00 - 16:30 Hrs <br />PM: 88°F Clear <br />The Contractor completed excavation of the 3x3 ft liner anchor trench on the north slope using an excavator <br />and a spotter. <br />• The Contractor completed subgrade work along the north and west synthetic liner tie-in; used laborers with <br />rakes and shovels and a motor grader; removed loose soil with small 623 Cat scraper. <br />• The Contractor performed grading work in west side of the clay borrow site. <br />• The Contractor continued moisture conditioning of haul roads and adjacent areas for dust control. <br />Personnel: (1) Superintendent, (1) Grade Checker, (4) Operators, (3) laborers. <br />Equipment: (1) 825 Cat Sheepsfoot compactor, (1) 140H Cat Motor -Grader, (2) Water trucks, (1) Cat Excavator, <br />(3) D6 Cat Dozers, (5) 637 Cat Scrapers, (1) Cat Fork-lift. <br />CQA (GLA) Activities: Field Personnel (1) <br />• CQA personnel performed continuous monitoring of all construction activities insuring conformance with the <br />project specifications. <br />CQA personnel monitored subgrade work along the liner tie-in in FLI-13. <br />• CQA personnel verified liner anchor trench dimensions. <br />• CQA personnel continued visual and lab soil classification testing of clay soil material from borrow area. <br />Meetings and Site Discussions: <br />• Met with Kevin Turner (Guinn Superintendent) in the morning to discuss planned activity for the work shift. <br />The Contractor would continue efforts on the top of the west and north slopes subgrade and the liner anchor <br />trench excavation. <br />PAGE a OF 2 <br />
