Geo-LoqkASSOCIATES CQA DAILY SUMMARY REPORT #037 REPUBLIC SERVICES FORWARD LANDFILL, FU -13 • Comanco performed HDPE liner repairs. The liner repairs consisted of patches on "T" seams, welding equipment burn outs, air- pressure testing holes, and seam destructive testing locations. Personnel: 12 • Equipment: (1) Fork Lift, (1) equipment trailer, (1) Van, (3) Trucks CQA (GLA) Activities: Field Personnel (2) CQA personnel performed continuous monitoring and documentation of all construction activities insuring conformance with the project specifications. • CQA personnel continued monitoring the moisture conditioning of the exposed clay surface throughout the work -shift. • CQA personnel monitored the installation of the GCL at all times insuring proper handling and orientation of the GCL panels on the slope; performed visual inspection of GCL liner surface prior to the installation of the HDPE liner insuring conformance with the project specifications. • CQA personnel monitored installation of the HDPE liner at all times insuring that proper procedures were used to prevent damage to the underlying synthetic liner. CQA monitored and inspected all panel seams for• cleanliness, minimum overlap, and the implementation of proper seaming procedures. CQA personnel marked seam destructive samples of HDPE production seaming to -date. Seam destructive tests are performed on site by CQA personnel. CQA personnel monitored and documented non-destructive air pressure and vacuum testing of HDPE hot wedge fusion and extrusion welds. Meetings and Site Discussions: • Met with Jose Hernandez (Comanco Superintendent) in the morning to discuss planned activity for the work shift. The Contractor would continue installation of the GCL and the HDPE on the lower 4:1 slope.
