Geo-Losic <br />ASSOCIATE <br />CQA DAILY SUMMARY REPORT #038 <br />REPUBLIC SERVICES FORWARD LANDFILL, FU -13 <br />• Comanco performed HDPE liner repairs. The liner repairs consisted of patches on "T seams, welding <br />equipment burn outs, air- pressure testing holes, and seam destructive testing locations. <br />• Personnel: 12 <br />• Equipment: (2) Fork Lifts, (1) equipment trailer, (1) Van, (3) Trucks <br />CQA (GLA) Activities: Field Personnel (2) <br />• CQA personnel performed continuous monitoring and documentation of all construction activities insuring <br />conformance with the project specifications. <br />• CQA personnel continued monitoring the moisture conditioning of the exposed clay surface throughout the <br />work -shift. <br />• CQA personnel monitored the installation of the GCL at all times insuring proper handling and orientation of <br />the GCL panels on the slope; performed visual inspection of GCL liner surface prior to the installation of the <br />HDPE liner insuring conformance with the project specifications. <br />• CQA personnel monitored installation of the HDPE liner at all times insuring that proper procedures were <br />used to prevent damage to the underlying synthetic liner. CQA monitored and inspected all panel seams forle <br />cleanliness, minimum overlap, and the implementation of proper seaming procedures. <br />• CQA personnel marked seam destructive samples of HDPE production seaming to -date. Seam destructive <br />tests are performed on site by CQA personnel. <br />• CQA personnel monitored and documented non-destructive air pressure and vacuum testing of HDPE hot <br />wedge fusion and extrusion welds. <br />Meetings and Site Discussions: <br />• Met with Kevin Turner (Guinn Construction Superintendent) and Jose Hernandez (Comanco Superintendent) <br />in the morning to discuss planned activity for the work shift. Comanco would continue installation of the GCL <br />and the HDPE on the lower 4:1 slope. Comanco would plan the installation of the GCL and the HDPE <br />considering expected high temperatures in the afternoon. <br />• Guinn Construction would perform grading work on clay surface; would continue moisture conditioning <br />exposed clay surface. <br />Site Visitors: <br />• None • <br />PAGE 2OF3 <br />
