Submitted to: Scott Purdy (GLA) Project #: 2013-AO55 Report No.: 068 Report Date: Wednesday August 14, 2013 Onsite: 06:00 - 16:30 Hrs Weather: AM: 61°F Clear PM: 98°F Clear Guinn Construction Activities: • The Contractor completed placement of the operations soil layer on the west slope bench and along the lower north and west slopes, ten (10) vertical feet on the slope. The Contractor used two (2) LGP D6 Cat Dozers equipped with GPS to spread the OPS layer in a continuous 12 -inch lift on the slope and 24 -inches thick on the bench. • The OPS material was transferred from the borrow area located SW of the Cell with a 637 Cat scraper and a Cat 740 rock truck. Personnel: (4) Operators, (1) Supervisor, (1) Grade checker. Equipment: (1) Water truck, (2) 637 Cat Scrapers, (1) D6N Cat Dozer, (1) D6R LGP Cat Dozer, (1) 966 Cat Loader, (1) 14H Cat Motor grader, (1) Cat 740 Rock truck CQA (GLA) Activities: Field Personnel (1) • CQA personnel performed continuous monitoring of all construction activities ensuring conformance with the project specifications. • CQA monitored the placement of the OPS layer at all times ensuring that proper lift thickness was maintained and that care was taken to prevent damage to the underlying synthetic liner. Meetings and Site Discussions: • Met with Kevin (Guinn Superintendent) in the morning to discuss planned activity for the work shift. The Contractor would complete OPS layer placement today. . CQA personnel attended the last weekly project progress meeting held in the front entrance facility at 1000 hrs; refer to the weekly project progress meeting minutes.
