When to Apply <br />Facility operators of existing facilities must file an NOI in <br />accordance with these instructions by March 30, 1992. Facility <br />operators of new facilities (those beginning operations after <br />March 30, 1992) must file an NOI in accordance with these <br />instructions at least 14 days prior to the beginning of operations. <br />Once the completed HOT, site reap, and appropriate tee have been <br />submitted to the State Water Board, your NOT will be processed and <br />you will be issued a receipt letter with a Waste Discharge <br />Identification (WDID) Number. Please refer to this number when you <br />contact either the State or Regional Water Beards. <br />Lag <br />The total annual fee is $830. Checks should be rade payable to: <br />SWPCEl, <br />Change of Information <br />If the information provided on the MOI or site map changes, you <br />shaild report the changes to the State Water Eoard using an NOT <br />form. Please complete <br />the NOT form in its entirety and sign the certification. <br />Section I--NOI STATUS <br />Check, box "A" if this is a new NOT registration. <br />Check box "E" if you are reporting changes to the NOT (e.g., new, <br />contact person, phone number, mailing address). Include the <br />facility WDID V Highlight all the information that has been <br />changed. <br />Please note that a change of information does not apply to a change <br />of facility operator or a change in the location of the <br />facility. These changes require a Notice of Termination (NOT) and <br />submittal of a new 001 and annual fee. Contact the State Water <br />Board or Regional Plater Boards for more information on the NOT Form <br />and instructions. <br />
