The design and operation of this facility must comply with all applicable State N"Ltimum Standards for solid eva_te handling as <br />specified in Title 14, California Code of Regulations. <br />2. The operator shall maintain a lo-, of special/unusual occurrences. This toy shall include, but is not limited to, fires, explosions, <br />the discharge and disposition of hazardous or un -permitted wastes, and significant biiuries, accidents or property damage. Each <br />log entry shalt be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The log shall be <br />avai able to site personnel and the EA at all times. <br />3. The operator shall comply with all waste discharge requirements, Clean-up and Abatement Orders, monitoring, remediation <br />schedules and related requirements of the local California Regional Water Qualin Control Board (CRWQCB). Doomdation of <br />water connected to this site shall be remediated in the manner specified y the CRWQCB. <br />4. Additional information concerning the design and operation of this facility must b: furnished upon the request of the LEA <br />5. Fonsard Resource Recovery Facility mint comply with the conditions of approval of Use Permit UP -00-07. <br />6. The LEA reserves the right to suspend or modify waste receiving and handling operations «hen deemed necessary due to an <br />emergence. a potential health hazard, or the creation of a public nuisance. <br />7. Any chance that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is <br />prohibited Such a change may be considered a significant change, requiring a permit revision. In no case shall the operator <br />implement any change without first submitting a written rotice of the proposed change, in the form of ars RFl amendment, to the <br />LEA at Last 180 days in advance of the change. <br />S. The facilit} shall meet the design and operational standards of the California Code of Reuulations, Tile i 4 (CCR 14). Sections <br />17400-17564, and Title 14 (CCR 14). Sections 17850 17870, <br />10. All stockpiling of compostable organic material for a combined period of time greater than six months is considered disposal and <br />must be either removed from the site or processed for composting operations. <br />13. Wastewater shall not be allowed to discharge off the property or into any waterway except as allowed by NPDES permit. <br />14. All incoming waste shall be inspected for hazy dour waste. if ha -1 dour waste is found it shall be handled as hazardous waste in <br />accordance with all applicable State standards. <br />15. In accordance with Title 14 (CCR 14), Section 17867.5. Operators shall ensure that all personnel assigned to the operation shall <br />be trained in subjects pertinent to operations and maintenance, physical contaminants and hazardous materials recognition and <br />screening, with emphasis on odor impact management and emergency procedures. A record of such training shall be maintained <br />on the site. All personnel working with the load screening program shall be trained to recognize hazardous wastes, and shall be <br />trained for safety and evacuation procedure <br />16. This permit is subject to review by the LEA and may be modified, suspended, or revoked for sufficient cause following a hearing. <br />17. Wastes to be composted must go directly into composting operation and may not be stockpiled on site for more than seventy-two <br />(72) hours. <br />18. Leachate control is required. <br />19.
