Construction Quality Assurance(CQA) Plan Extension of Closure Cap <br /> Corral Hollow Sanitary Landfill August 21, 2008 <br /> ♦ A summary of deficiencies and/or defects, and resolutions. <br /> ♦ Ongoing summary of changed and/or change orders to the work. <br /> ♦ The signature of the CQA Manager. <br /> ♦ On the fourth week of each month the report will include a summary of <br /> on-site and third party laboratory test results. <br /> 5.3 Photographs <br /> Construction activities will be photographed. Photographs will include any significant <br /> problems encountered and corrective actions, and will document construction progress. <br /> The photographs will be identified by number, location, time, date, and photographer. <br /> The photographer will document the subject or the photograph, either on the back of <br /> the picture, or in a photograph log. <br /> 5.4 Design and Specification Changes <br /> Design and specification changes may be required during construction. Design and <br /> specification changes will only be made with written agreement between the Owner <br /> and Contractor. These changes will be made by change order to the contract. When <br /> change orders are issued, they will be prepared by the construction manager. The <br /> construction manager will distribute change orders for signature and execution to the <br /> required parties. The CQA Manager will include inspection of these changes in he <br /> CQA scope of work. <br /> 5.5 Fina/Construction Report <br /> At the completion of the project, the CQA Manager and CQA Officer will submit a final <br /> construction report (FCR) to the Owner.
