® Verify that adequate anchor trenches are constructed. <br /> The CQA Monitor shall inforin the CQA Manager and the Resident Engineer if the above <br /> conditions are not met. The Resident Engineer will notify the Contractor. If the Resident <br /> Engineer is not available, the CQA Monitor will notify the Contractor and the County of <br /> apparent non-conformance. <br /> 5.1.4 Deficiencies <br /> When deficiencies (items that do not meet specified values) are discovered, the CQA Monitor <br /> shall immediately determine the nature and extent of the problem, notify the CQA Monitor and <br /> Engineer, and complete required documentation. The Engineer will notify the Contractor within <br /> one hour after discovery of deficiency, or on the next working day if discovered at the end of a <br /> shift. If the Engineer is not available, the CQA Monitor shall notify the Contractor and the <br /> County of apparent non-conformance. <br /> The Contractor's qualified representative(s) will be required to correct the deficiency to the <br /> satisfaction of the Engineer. If the Contractor is unable to correct the problem,the CQA Monitor <br /> will develop and present suggested solutions to the Engineer and the County for their approval. <br /> If the solution requires a design modification,the Engineer shall also be contacted as-needed. <br /> The corrected deficiency shall be retested before additional work is performed by the Contractor. <br /> All retests and the steps taken to correct the problem shall be documented by the CQA Monitor. <br /> 5.2 Geomembrane Quality Assurance <br /> The specifications for the geomembrane are provided in Section 02779 of the project <br /> specification. The specifications contain the tabulated summary of testing required and test <br /> frequencies described herein. <br /> ' 5.2.1 Delivery <br /> Upon delivery of geomembrane,the CQA monitor must: <br /> ® Inspect geomembrane rolls for damage potentially occurring during shipping and/or <br /> handling, then identify damaged materials and verify damaged materials are set aside and <br /> not installed. <br /> ® Verify the geomembrane is stored in accordance with the technical specifications and is <br /> ' protected from puncture, dirt, grease, water, moisture, mud, mechanical abrasions, <br /> excessive heat, direct sunlight,and other damage. <br /> ® Verify each roll is marked or tagged with manufacturer's name; project identification; lot <br /> number; roll number; roll dimensions and this information is documented on a <br /> geosynthetic receipt form. <br /> CH CQA Plan 041712 Rev2a.doc 5-3 Construction Quality Assurance Plan <br />
