COMPLIAIq.LETATEMENTS FOR DISPOSAL.LIIE, STANDARDS 17,-2 <br /> Comply with all circled violations-, <br /> (ED Incoming records of weights and volumes to within 10% accuracy were <br /> not instituted by the site operator. <br /> 0537 Subsurface records were not maintained on the length and depth of <br /> any cuts made in the natural terrain where fill will be placed. <br /> CaEDA location of all accurate records pertaining to the landfill <br /> operation were not provided to the San Joaquin Local Health District. <br /> 17646 An adequate number of trained personnel were not available at the <br /> site to monitor and direct incoming vehicles , provide equipment <br /> maintenance, insure safety and apply environmental controls or <br /> emergency procedures .. <br /> 17647 Personnel assigned to operate the site were not adequately trained <br /> in site operations , maintenance, safety, environmental controls or <br /> emergency procedures . <br /> 1768 Adequate supervision was .not provided at the site to insure proper <br /> operation of the site incompliance with all laws , regulations or <br /> permit conditions . <br /> 17648 Names , addresses , telephone numbers and other emergency, information <br /> was not provided at the disposal site. <br /> 17656 Identification signs were not provided at each point of access to <br /> the disposal site indicating the name of the site operator. <br /> 17657 Entry signs at each point of access indicating a schedule of charges , <br /> hours of operation, and types of materials accepted, were not provided <br /> by the operator. <br /> 17658 Adequate barriers were not provided around the perimeter of the <br /> disposal site. <br /> 17659 Access roads to the disposal site were not smooth and designed to <br /> minimize the generation of dust and the tracking of mud during <br /> inclement weather conditions . <br /> 17660 Internal roads used by the public or commercial vehicles were not <br /> maintained in a safe condition or maintained such that vehicle <br /> access and unloading can proceed safely during inclement weather <br /> conditions . <br />
