Site hazards - Physical hazards, chemical hazards, electrical hazards, mechanical <br /> hazards, biohazards, heat stress, and noise hazards (see Section 3.2 for further detail). <br /> b. PPE selection- Level D protection during all tasks unless air monitoring dictates the use of <br /> Level C respiratory equipment. <br /> c. PPE use - Level D protection will be utilized for all tasks [drilling, well installation, and <br /> sampling]. <br /> d. Duration of Site operations-Approximately 3 to 6 months. <br /> e. PPE maintenance and storage -The contractor will be responsible for the maintenance <br /> and storage of personal PPE. <br /> f. PPE decontamination - All contaminated disposable clothing (e.g. gloves) will be <br /> properly bagged and disposed of accordingly. All personnel will be instructed to wash <br /> hands, face, neck, and forearms at the end of the work shift and to shower at the end of <br /> each workday (see Section 9 for further details). <br /> g. PPE training and proper fit - All field personnel will have the required HAZWOPER <br /> certification and on the job experience/training. Therefore, all field personnel will have <br /> the knowledge of proper PPE training and understand the proper fitting of the required <br /> PPE level (see Section 4.1 for further details) <br /> h. PPE dressing procedures - As stated in above, all field personnel will have the required <br /> certification and training. Therefore, all field personnel will have the knowledge of the <br /> proper PPE dressing (on and off) procedures. <br /> i. PPE inspection - Daily inspections will be conducted to verify the PPE is utilized and <br /> utilized correctly. The inspection will be recorded on the Site safety checklist provided in <br /> Appendix C. <br /> j. PPE in-use monitoring - All PPE will be monitored by field personnel throughout the day. <br /> Immediate response will be taken when monitoring indicates PPE replacement or <br /> adjustments are necessary (e.g. ripped gloves, improper fit) <br /> k. Evaluation of program effectiveness - The results of the PPE in-use monitoring and daily <br /> inspections will assist in determining the overall program effectiveness. Any issues will be <br /> addressed and changes will be implemented to the program incorporated into the daily <br /> Site protocol. <br /> I. Temperature limitations - Heat stress is a potential hazard on-Site.
