Liberty Rd. will need to be completed. The first attachment is a word document. It is the new Notification Form that you must complete. The second attachment is a PDF. It is a scanned copy of the current Notification Form you completed in the year 2000. I've attached the scanned current Notification Form for you to reference when completing the new form. One thing that must be changed on the new Notification Form is the annual loading. On the current form you have 10,000 cubic yards. Under a notification tier, you are allowed only 1,000 cubic yards. Also an updated Odor Impact Minimization Plan (GIMP) needs to be completed and submitted. I've attached your current OIMP (PDF; third attachment) to be used as a reference. I've also attached the regulation requirements for the OIMP (word; fourth attachment) to also be used as a reference. There is also the option of inactivating the composting permit for that location. If you choose to do so, you do not need to submit an updated Notification Form or GIMP. Below is my contact information if you have any questions.
