Liberty Rd. will need to be <br />completed. The first attachment is a word document. It is the new Notification Form that you must <br />complete. The second attachment is a PDF. It is a scanned copy of the current Notification Form you <br />completed in the year 2000. I've attached the scanned current Notification Form for you to reference <br />when completing the new form. One thing that must be changed on the new Notification Form is the <br />annual loading. On the current form you have 10,000 cubic yards. Under a notification tier, you are <br />allowed only 1,000 cubic yards. <br />Also an updated Odor Impact Minimization Plan (GIMP) needs to be completed and submitted. I've <br />attached your current OIMP (PDF; third attachment) to be used as a reference. I've also attached the <br />regulation requirements for the OIMP (word; fourth attachment) to also be used as a reference. <br />There is also the option of inactivating the composting permit for that location. If you choose to do so, <br />you do not need to submit an updated Notification Form or GIMP. Below is my contact information if <br />you have any questions. If you choose to keep the permit active, please submit to me these required <br />documents. Thanks. <br />
