San Joaquin valley <br />Air Pollution Control District <br />Permit Renewal and Emission Inventory Questionnaire <br />Attached are questionnaire forms for annual process data for equipment that have <br />received a Permits to Operate (PTO) from the District. The purpose of requesting this <br />information is to update your facility records with the District, help expedite the renewal of <br />your company's permit(s), and to collect data for the District's and State's emission <br />inventories. <br />The information, which you supply to the District, must represent your operation for <br />calendar year 2001 only. Please note, the attached questionnaires are general in nature <br />and not all questions on each questionnaire may apply to the permitted unit(s) listed, only <br />f answer those questions that are applicable.. Should there be inaccuracies _ in the <br />equipment description or other information, please indicate your correction on the <br />questionnaire before returning it to the District. <br />Please return the questionnaire by April 22, 2002 to Ms. Ester Davila at anyone of the <br />addresses below.
