en iro- ,t 7/id_ Is <br />The San Joaquin County Environmental Health Department (EHD) received the revised <br />Five -Year Permit Review Application Package for the Lovelace Transfer Station (39 -AA - <br />0008). Upon review the EHD has made the following comments and findings listed below: <br />1. Application for Solid Waste Facility Permit: <br />- Page 3, Part 6 A — Required with all application submittals — Update TPR date. <br />2. TPR - Certification — Missing signature. <br />3. TPR - Index: <br />- Page vi — Drainage Control, Load Checking and Visual Screening — Needs <br />corrections on page number. <br />- Page vii — Recycling Center Design — Needs corrections on page number. <br />- Page 1-1.2 — Purpose — TPR replaces the 2010 Transfer Processing Report. <br />Needs corrections. <br />- Page 16 - 2.8 — Provisions for Peak Loading - Data used for 2009. Needs <br />corrections. <br />- Page 25 - Facility no longer uses Golden By -Product to transported <br />waste/used tires off-site. Needs corrections. <br />5. Appendix B: <br />- Lathrop -Manteca Fire District Permit No: 09-123 has been expired on 5/31/2010. <br />Replace with current permit. <br />- CIWMB Certificate expired on 6/5/2010.
